OAP died after drowning in a channel where the investigating judge issued an urgent warning to council officials.

Doreen Turner, 91, lost her tragic life after driving her car to the body of the water in Chichester and West Sussex.

Horror crashes occurred on November 1 of last year and sparked an investigation into the police.

The Criminal Investigative Judge has now ruled that the West Sussex County Council must prove more safety measures along the way.

A probe found that there was no criminal evidence of any mechanical problems in Doreen.

The officers also concluded that the 91 -year -old did not suffer from a weakness that would affect her leadership.

However, curbing was found at the end of the road to measure shorter than the standard height requirements.

“We are currently reviewing this site in consultation with the Sussex police and we will respond to the investigator accordingly,” said a spokesman for the council.

The pathologist Joan Andrews could not determine the exact reason that made Doreen had left the road.

But it warned the council, “action must be taken.”

“In my opinion, action should be taken to prevent future deaths and I think your organization has the ability to take such a measure,” she said.

This incident represents the second dead accident in the same area during the past five years.

The investigator said that the investigation heard that “there are no devices that prevent a vehicle that passes over Kerbstons from entering the channel.”

The West Sussex County Council until June 25 to formally respond to the concerns of the Criminal Investigation Judge.

A teenage boy died tragicly after jumping to a channel for the first time this month.

Delvin Musakwa, 13, was pulled from water after jumping to the channel in Exter, Devon, on May 5.

The police said the death was not treated as suspicious, adding that it would prepare a file for the roads.
